Key Responsibilities
The successful candidate will:
Provide comprehensive assessment, diagnosis, treatment, and follow-up for patients with chronic illnesses including hypertension, diabetes, cardiovascular diseases, asthma, COPD, chronic kidney disease, obesity, and related conditions;
Develop individualized management plans and monitor treatment outcomes using evidence-based clinical guidelines;
Provide patient education and counselling on lifestyle modification, medication adherence, and disease self-management;
Coordinate referrals and multidisciplinary care for patients requiring specialist input;
Participate in clinical audits, quality improvement initiatives, and development of NCD clinical protocols;
Maintain accurate clinical documentation within the hospital electronic medical records system;
Mentor nursing staff and support continuous professional development activities within the NCD clinic.
Minimum Qualifications
Applicants must have:
Diploma in Clinical Medicine and Surgery from a recognized institution.
Current registration and valid practicing license with the Clinical Officers Council (COC) of Kenya.
Postgraduate qualification in Non-Communicable Disease (NCD) Management from a recognized institution.
At least three (3) years' experience in managing patients with non-communicable diseases in a hospital or specialized outpatient setting.
Strong clinical assessment, communication, and patient counselling skills.
